If you answered yes to both of these questions then you need to complete the Parent Declaration Form. This form officially documents and declares a change of address that will result in the transfer of your player to a new association. NOTE: If you are a player living in another association's jurisdiction but are trying out for or have been selected to play on a rep team in Truro, you do not need to complete this form. You will be asked to register with us, once team selection has been finalized.
