Have you recently moved to the Truro area?

Did your child play hockey last season with another Association?

If you answered yes to both of these questions then you need to complete the Parent Declaration Form. This form officially documents and declares a change of address that will result in the transfer of your player to a new association. Please complete this form and send it to our Office Administrator at trurominorhockey@gmail.com.

NOTE: If you are a player living in another association's jurisdiction but are trying out for or have been selected to play on a rep team in Truro, you do not need to complete this form. You will be asked to register with us, once team selection has been finalized.
